The submission of the patient forms is through a secured network.
These forms require Adobe Acrobat Reader to view. If you do not have Adobe Reader already installed on your computer, click the Adobe logo to download.
If you are using Mac OS X and Safari browser, please make sure that you have the latest version of Adobe Reader installed.
Open Adobe Reader and select "Preferences" from the "Adobe Reader" menu. Make sure that the option "Display PDF in browser using:" is checked. Then you will be able to view and submit the above forms.
